All post-acute performance data in this report is derived from CMS Medicare Fee-for-Service (FFS) Claims. This is the same claims data used in CMS star ratings, value-based purchasing programs, and public quality reporting. Data reflects discharges from Ascension National hospitals to skilled nursing facilities during the period Q1 2025 – Q4 2025 (CMS Medicare FFS Claims).

Facility Hospitalization Rate (FSHR)

FSHR (Facility Hospitalization Rate) measures the percentage of SNF patient-days that result in an acute hospital readmission, calculated at the SNF-hospital relationship level. Unlike simple 30-day readmission rates, FSHR captures rehospitalization across the full SNF stay, providing a more comprehensive view of post-acute performance. The Ascension National system-wide weighted average FSHR is 35.5% for Q1 2025 – Q4 2025 (CMS Medicare FFS Claims). For context, the Florida state average 30-day SNF readmission rate is approximately 17.1% and Alabama’s is approximately 15.4% — FSHR is a distinct metric that covers the full SNF stay rather than a fixed 30-day window, so system-average FSHR is the primary internal reference throughout this portal.

Priority Score Methodology

Priority scores combine three factors: (1) FSHR magnitude above the system average, (2) total patient volume at the facility, and (3) multi-hospital presence within Ascension National. SNFs appearing at 2+ hospitals receive a multiplier reflecting their system-wide engagement leverage. Scores are relative within the Ascension National system.

CMS Suppression & Data Availability

CMS suppresses FSHR values for SNF-hospital relationships with fewer than 11 patients in the measurement period. Of Ascension National’s 557 unique SNF destinations, only those with sufficient volume carry FSHR data. Downtown Ascension: CCN 100006 aggregates ORMC plus seven subpart campuses (Arnold Palmer, Winnie Palmer, Dr. P. Phillips, Jewett Orthopedic, Cancer Institute Downtown, Horizon West, Lake Mary) into one CMS billing record per AHD.com, so the ORMC tile represents the combined downtown SNF-discharge footprint. Sebastian River Hospital (Steward acquisition, Oct 2024) is an acknowledged FSHR exception: patient volume is captured but every top SNF destination row returned suppressed FSHR values in the current CMS FFS pull, a legacy Steward-billing artifact. “Suppressed” SNF destinations are counted but not rated in this analysis. All 38 of 38 Ascension National CCN-registered hospitals drive measurable FFS post-acute data.

FSHR Color Coding

Throughout this portal, FSHR values are color-coded relative to the Ascension National system average (35.5%): Red = ≥40.5% (5+ pp above system); Amber = ≥37.5% (2+ pp above); Green = ≤32.5% (3+ pp below). Uncolored values are within ±2–3 pp of system average.
